CMS is implementing the new CMS1500 (08-05) claim form on Janary 1, 2007, but the form will not be mandatory until June 1, 2007. The major difference between the new CMS1500 (08-05) form and the prior CMS1500 (12-90) form is the split provider identifier fields.
The split fields will enable NPI reporting in the fields labeled as NPI, and corresponding legacy number reporting in the unlabeled block above each NPI field. In addition, the bar code has been dropped from all versions of the form.

What is the difference in the available formats? "Computer" forms have tear off sides and produce a duplicate copy. "Laser Sheets" are used with a desk jet or ink jet printer.
